Requirements

  • A copy of the identity document of the contracting party, enrollee or legal representative, if applicable
  • A copy of the minor's identity document
  • A copy of the birth certificate (later it must be regularized with a copy of the identity document, mandatory)
  • Payment of the registration fee and the first monthly premium
  • If you want to sign up for automated payments with your debit card by Visa, the cardholder must present a copy of the card to be used for debiting the premium and fill out a form
